Nomination and Application Form

Conditions
1. The following must be provided upon application:
- An application form signed by both the nominee and the nominator.
- A photocopy of the nominee's ID card.
- Two recent coloured passport photos of the nominee with his/her name clearly stated on them.
- A copy of the nominee's full CV.
- A printed portfolio representative of your best work and also 2 sets in CD, VCD, or DVD in a PowerPoint format.
2. Nominees must attend any interview arranged and provide any necessary documents.
3. All completed forms must be forwarded to Hong Kong Communication Art Centre before 6pm on the 16th June 2014 by hand or by mail before the set deadline.
Nominators' Criteria
- The nominator must be 21 or above.
- There is no limited number of nominations
- However, nominators must not participate in the contest.
Nominees' Criteria
- Must be 21 or above.
- Must be nominated by an eligible nominator.
- Must possess at least 3 years of full-time work experience at the design industry
Preliminary Round
- The Preliminary Round will be held between June and July 2014.
- The relevant information concerning nominees will be processed by Hong Kong Communication Art Centre to confirm their eligibility.
- Results will be transferred to the panel for
final judgment.
Announcement of the Results
Results will be announced in mid-August. Winning designs will be displayed during the period of the Hong Kong Art &
Design Festival which will be held in 8-10 August 2014 at the Hong Kong Convention and Exhibition Centre. Awards will be presented in 9th August 2014.
